Prime Care Life is a Targeted Case Management provider in Central Florida. We help children, families, adults, and seniors get connected to the care and resources they need — and we stay with them until things start moving.
Targeted Case Management, in plain terms.
Getting help shouldn't require knowing which agency handles what, which form to file, or who to call when a waitlist stalls. A case manager takes that on with you. They learn your situation, keep track of every piece, make the calls, and follow up — so the burden of coordinating it all isn't sitting on you alone.
We don't provide medical or clinical treatment ourselves. What we do is connect you to the people who can, and make sure your care stays organized and moving once you're there.
