Let's figure this out — together.

You're not the first

You probably didn't plan on being here today.

Most people we work with reached out in a moment they weren't expecting. If any of these sound familiar — you're in the right place.

You're facing housing instability or trying to access benefits

Housing programs, SSI, disability, food assistance — we help you find the right doors and actually get through them.

A doctor, school, or hospital just mentioned case management

You're not sure what that even means, or where to start. We can walk you through the whole thing in plain language.

You're worried about a child facing abuse or neglect

We coordinate safety planning, counseling, school support, and the services a child needs to be safe and thrive.

You're a case worker or provider looking for a partner

We work regularly with hospitals, pediatricians, DCF, and schools. Clean documentation, direct communication.

What to expect

Three steps.

A lot of the anxiety around asking for help is not knowing what happens next. Here's exactly how the first two weeks with PCL go — start to finish.

1
You reach out

Call, email, or use the form. A member of our intake team will respond.

2
Meet your case manager

An initial visit — at your home, or virtually. We listen more than we talk. This is where the plan actually gets built.

3
Ongoing support & advocacy

Regular check-ins, advocacy at appointments, and coordination between providers — for as long as you need us.

Our services

One team, every system you're navigating.

When we were about to lose our housing, PCL helped us find assistance and walked us through every application. They didn't just hand us a list — they stayed with us until it was handled.

Navigating SSI felt impossible until I had someone in my corner. My case manager knew exactly who to call and what to say.
David T. Client · Kissimmee, FL
As a discharge planner, I refer to PCL because I know families will actually get through to somebody. Clean documentation, direct communication, no chasing.

Common questions

Answers to what people usually ask first.

Some of the most common questions we hear on that first call. Nothing off-limits.

No. You can contact us directly. A referral from a doctor, hospital, or agency is welcome but not required — we'll help you gather anything we need on our end.
Coordination of care and resources across providers and agencies — housing, benefits, medical, behavioral health, school — plus advocacy at appointments, crisis planning, and regular check-ins. It's practical, day-to-day support.
Yes. We help with housing assistance, SSI and disability, food and transportation, employment and education resources, and more — alongside coordinating any mental or behavioral health care you need.
We have self-pay options and can also help you determine if you qualify for Medicaid or other programs. Call and we'll walk through what's possible.
Both. We offer in-home visits, virtual sessions, and phone support. We meet you where it's easiest.
Yes. Information is only shared with people you authorize, with limited exceptions required by Florida law (such as imminent safety concerns).
Ready when you are

Whenever you're ready. We're here.

The first step is a conversation. No forms, no commitments — just a member of our team on the other end of the phone.
