Who we serve

Care that meets you where you are.

A child, a teenager, an adult managing a lot at once, a family in transition — the needs are different at every stage, and so is the support. We shape our case management around the person in front of us.

Every stage of life

What you need depends on where you are.

What a young child needs looks nothing like what a senior needs, and a family working through a crisis needs something different again. Below is a closer look at the people we work with most — but the throughline is the same for everyone: one steady point of contact who keeps the whole picture in view.

Children

Children

Ages 0 – 12

For young children, safety and stability come first. We coordinate with schools, pediatricians, and — when a child's welfare is at stake — DCF, while making sure the adults around them have the support they need to keep them safe.

Teens and young adults

Teens & young adults

Ages 13 – 25

The years between childhood and independence bring real transitions — school, mental health, first jobs, benefits, sometimes a first place of their own. We help young people line up the support that keeps them moving forward instead of slipping through the cracks.

Adults and seniors

Ages 18+

Adults and older adults often reach us juggling several things at once — housing, benefits, medical appointments, disability or SSI paperwork. A case manager keeps all of it tracked and moving, so nothing important gets lost in the shuffle.

Households in transition

When a whole household is in transition, everyone feels it. We work with parents, caregivers, and children together — often alongside DCF — to steady the situation and connect the family to what it needs to move forward.

However you come to us

The situation matters more than the label.

People find their way to us from all kinds of circumstances — someone facing homelessness, someone managing a serious health condition, a family in the middle of a crisis. You don't need to fit a category to reach out.

Whatever brought you here, the work is the same: one person who learns your situation, keeps track of every piece, and helps you move through the systems around it.

The first step is a phone call — just a member of our team on the other end. No forms upfront, no commitments.
