One person who takes it on with you.
Help, in our world, isn't a pamphlet or a phone number to try. It's a case manager who learns your situation, coordinates the moving parts, and stays with you as things change.
What working with us actually involves.
There are two things worth knowing before you reach out: the areas we focus on, and how the first couple of weeks tend to go. Both are below — the focus areas first, then the process from your first call onward.
One team, every system you're navigating.
Targeted Case Management means we don't just point you to resources — we help you reach them, coordinate them, and keep them working together.

From your first call onward.
A lot of the anxiety around asking for help is not knowing what happens next. Here's how the first couple of weeks with Prime Care Life go — start to finish.
You reach out
Call, email, or use the form. A member of our intake team responds — usually within a few hours.
Meet your case manager
An initial visit — at your home, or virtually. We listen more than we talk. This is where the plan actually gets built.
Ongoing support & advocacy
Regular check-ins, advocacy at appointments, and coordination between providers — for as long as you need us.
Start with a conversation.
The first step is a phone call — just a member of our team on the other end. No forms upfront, no commitments.
